Welcome to the AHC Pre-Appointment Form provided by HallMark Veterinary & Compliance Services. If you do not agree with these terms, please do not use the AHC Pre-Appointment Form.

  1. Appointment Booking: The AHC Pre-Appointment Form is designed to facilitate the booking of appointments for the issuance of Animal Health Certificates. By filling out the form, you are requesting an appointment for this purpose. Appointment availability is subject to scheduling and capacity constraints.

  4. Payment: Payment for AHC services must be made in accordance with our payment terms. Failure to make payment may result in the cancellation of your appointment.
