Welcome to the AHC Pre-Appointment Form provided by HallMark Veterinary & Compliance Services. By accessing and using this form, you agree to comply with and be bound by the following additional terms and conditions. If you do not agree with these terms, please do not use the AHC Pre-Appointment Form.
Appointment Booking: The AHC Pre-Appointment Form is designed to facilitate the booking of appointments for the issuance of Animal Health Certificates. By filling out the form, you are requesting an appointment for this purpose. Appointment availability is subject to scheduling and capacity constraints.
Accuracy of Information: You agree to provide accurate and complete information in the form, including details about yourself, your pet(s), travel plans, and any supporting documents. Providing false or misleading information may result in the cancellation of your appointment.
Appointment Confirmation: Submission of the form does not guarantee an appointment. Appointment availability is subject to scheduling and capacity constraints. We will contact you to confirm your appointment details.
Payment: Payment for AHC services must be made in accordance with our payment terms. Failure to make payment may result in the cancellation of your appointment.
