Additional Terms and Conditions for AHC Pre-Appointment Form

Welcome to the AHC Pre-Appointment Form provided by HallMark Veterinary & Compliance Services. By accessing and using this form, you agree to comply with and be bound by the following additional terms and conditions. If you do not agree with these terms, please do not use the AHC Pre-Appointment Form.

  1. Appointment Booking

    The AHC Pre-Appointment Form is designed to facilitate the booking of appointments for the issuance of AHCs. Filling out the form constitutes a request for an appointment. Appointment availability is subject to scheduling and capacity constraints. Appointments will only be confirmed once payment has been completed.

  2. Accuracy of Information

    You are responsible for providing accurate and complete information in the form, including details about yourself, your pet(s), travel plans, and any supporting documents. Providing false, misleading, or incomplete information may result in the cancellation of your appointment or errors in the AHC.

  3. Rectification of Errors

    It is your responsibility to review the completed AHC upon receipt and report any inaccuracies immediately.

    • Corrections Due to HallMark Errors: Errors attributable to HallMark will be corrected at no cost, provided they are reported within 48 hours of receipt of the certificate and before your departure date.

    • Corrections Due to Client Errors: If errors result from inaccurate or incomplete information provided by the client, additional fees may apply.

    HallMark accepts no liability for errors not reported prior to travel or for delays/disruptions caused as a result.

  4. Acknowledgment of Service Nature

    AHCs are issued based on the information provided by the client and verified to the best of our ability. Final acceptance of AHCs is at the discretion of travel authorities, and HallMark is not liable for issues arising from changes in regulations or interpretations by border officials.

  5. Appointment Confirmation

    Submission of the Pre-Appointment Form does not confirm your appointment. To secure an appointment, you must:

    1. Select an available time slot.

    2. Complete payment in full.

    Once payment is completed, you will receive a confirmation email with your appointment details.

  6. Payment

    Full payment is required at the time of booking to confirm your appointment. Appointments cannot be secured without payment. If payment is not completed, no appointment will be scheduled, and availability may change.

  7. Appointment Changes and Cancellations

    If you need to reschedule or cancel your appointment, you must use the link provided in your appointment confirmation email. This email also includes an option to add the appointment to your calendar for convenience.

    • Rescheduling: Clients can reschedule appointments up to 48 hours in advance.

    • Cancellations: Appointments cannot be canceled or rescheduled less than 24 hours before the scheduled time.

    HallMark reserves the right to charge a fee for late changes or cancellations.

  8. Documentation Requirements

    You must bring all necessary documentation to your appointment, including proof of ownership, vaccination records, and any other required paperwork. Failure to provide the necessary documentation may result in the cancellation of your appointment.

  9. Modification of Terms

    HallMark Veterinary & Compliance Services reserves the right to modify or update these terms and conditions at any time without prior notice. Any changes will be effective immediately upon posting on this page. It is your responsibility to review these terms periodically for updates or changes.

  10. Compliance with Regulations

    You agree to comply with all relevant regulations and requirements related to the issuance of AHCs, including but not limited to vaccination and microchipping requirements.
